In order to begin the admissions process to the Lutheran Home of Southbury, please complete an application. To obtain an application, stop by the Lutheran Home or contact our Director of Admissions by telephone at 203-264-9135. You may also email us.
Once we have received your completed application, our staff will review it thoroughly to ensure that we can provide the appropriate care required by the applicant. After the approval of an application, staff will contact the applicant and / or responsible party to discuss arrangements for admission. All admissions are based upon room availability.
Please call for daily private pay rates. Applicants who are eligible for Medicaid, Medicare or private insurance coverage may call to learn more about their benefits. Residents are also encouraged to ask about Evercare health care benefits that may cover various medical services. Admissions may be arranged from hospitals or from the community. Transfers from other nursing homes may also be arranged.
