Please Visit Us! The Lutheran Home of Southbury is conveniently located off I-84 at 990 Main Street North in Southbury, Connecticut. To reach our facility, take I-84 and get off at Exit 15. Proceed north on Main Street North (Route 6) for approximately 1 1/2 miles. You will see an oval sign for the Lutheran Home of Southbury on the right-hand corner of Main Street North and Dublin Hill Road. Turn right onto Dublin Hill Road. The Lutheran Home of Southbury is the expansive brick building on your left. Pass the Emergency Entrance and make your next left through the Main Entrance. When you arrive, please see our receptionist in the lobby, who will be happy to serve you, or ask for the Director of Admissions, Denise Cass. In order to begin the admissions process to the Lutheran Home of Southbury for Skilled Nursing, Rehabilitation Services, or Parley Manor independent living, please request and complete an application. To obtain an application, stop by the Lutheran Home or contact our Director of Admission at 203-264-9135 x 322; dcass@lhsouthbury.org. Once we have received a completed application, our staff will review it thoroughly to ensure that we can provide the appropriate care required by the applicant. After the approval of an application, staff will contact the applicant and/or designated family members to discuss arrangements for admission. All admissions are based upon room availability. Applicants who are eligible for Medicaid, Medicare or private insurance coverage may call to learn more about its benefits for residents. Long-term residents are also encouraged to ask about Evercare health care benefits, which cover various medical services. Admissions may be arranged from any area hospital or from the community. Transfers from other nursing homes may also be arranged.
