Short Term Rehabilitation

The Lutheran Home of Southbury Rehabilitation Program is a personal one-on-one approach to helping people with or without memory loss to achieve their highest practicable physical abilities.  Our therapists are employed by the Lutheran Home, not contracted from an outside rehabilitation company.  All have received advanced training in memory loss techniques during rehabilitation.

Lutheran Home of Southbury provides an inter-disciplinary rehabilitation program, helping people regain strength and dexterity after illness, surgery, or trauma.  Our small staff of caregivers provides personal service and develops a personal relationship with your loved one.

Rehabilitation services include:
  1. Physical Therapy
  2. Occupational Therapy
  3. Speech Therapy
  4. Community Re-Entry Support

Therapy expertise includes treatment for: orthopedic conditions, such as knee replacements, fractures, and chronic arthritis; neurological conditions, such as stroke; cardiac conditions, such as heart bypass surgery; pulmonary conditions, such as emphysema; diabetes; and post-operative recovery. All therapy programs are customized and developed in coordination with a resident’s doctor.

In addition to therapeutic care, rehab residents receive personal and social support. Therapists, social worker, nurses, and Certified Nursing Assistants provide comfort and reassurance. A comprehensive discharge plan is developed in conjunction with residents and their families that enables individuals to continue to thrive once they return to the community. Community re-entry plans may include in-home services, such as home healthcare workers, as well as out-patient therapy and exercise programs.

The Lutheran Home of Southbury is a Medicare/Medicaid certified facility. Other insurance plans may be accepted with pre-authorization. Rooms at the Lutheran Home of Southbury may be reserved and therapy services may be arranged before a hospital stay.
